Privacy Notice for T4A Information

This webform collects your SIN, name, and mailing address solely to issue your T4A slip, as required by the Canada Revenue Agency (CRA) under the Income Tax Act.

Your information will be used only for preparing, filing the T4A with CRA, and mailing your copy by the deadline, unless you agree below to emailing of the T4A slip.

Providing this information is voluntary, but required for T4A issuance.
