Community Impact Grant – Application Form


All CRNs are eligible to apply for up to $5,000 in Community Impact Grants annually!

  • Applications can be in any amount as long as the annual total does not exceed $5,000.
  • Multiple applications can be submitted per year to a maximum total of $5,000.
  • Grants are administered on an annual basis from April 1 to March 31.
  • CRNs may apply for grants of up to $5,000 from April 1 to November 30.
  • After November 30, grants of up to $2,500 are available.
  • The final deadline to apply for grants for the fiscal period is February 15. All funds must be expended and a Community Impact Grant Report & Invoice Form must be submitted by March 31.

To receive a Community Impact Grant for your CRN, please follow these steps:

Complete the Community Impact Grant Application Form (below), before you start your project.

  1. Be sure to review your project idea with your Regional Mentor before submitting.
  2. Once approved, you will receive an email confirmation from the Director of Administration.

After your project, complete the Community Impact Grant Report & Invoice Form.

Note: All Community Impact Grant Applications must be submitted using the online form. A pdf version of the Grant Application Form is available for planning purposes. Do not submit your application by email.
